MINI & ME PARENT + CHILD PROGRAM WAIVER
I understand that Mini & Me is a parent-participation program for children ages 2–5. I acknowledge that I (parent/guardian) must remain physically present and actively participate in every session for the safety of my child.
I understand soccer involves physical activity and there is inherent risk of minor injuries (falls, scrapes, bumps). I release Bravos Soccer Academy, Coach Gerardo Bravo, and all staff from liability for any injury sustained during normal program activity that is not caused by gross negligence.
I authorize Bravos Soccer Academy to seek emergency medical care for my child if I cannot be reached immediately, and I confirm that all emergency contact information I provided is accurate and current.
BSA UNITED COMPETITIVE LEAGUE WAIVER
I understand that BSA United is a competitive youth soccer league with regular practices and weekly matches. My child will participate in age-appropriate physical training, scrimmages, and competitive games under the supervision of Bravos Soccer Academy coaching staff.
I acknowledge the inherent risks of competitive soccer, including but not limited to: sprains, strains, fractures, concussions, and other physical injuries. I voluntarily assume these risks on behalf of my child and release Bravos Soccer Academy, Coach Gerardo Bravo, and all staff from liability for injuries sustained during normal program activity.
I authorize Bravos Soccer Academy to seek emergency medical care for my child if I cannot be reached immediately. I confirm all emergency contact and medical information provided is accurate and current, and I will notify the academy of any changes.
I understand that league fees include referees, uniform, and practice jersey. There are no traveling matches — all games are played locally.
1-ON-1 PRO SOCCER MASTERY WAIVER
I understand that 1-on-1 Pro Soccer Mastery is high-intensity personalized training with Coach Gerardo Bravo. Sessions include technical drills, tactical instruction, and physical conditioning tailored to the player's individual skill level and goals.
I acknowledge the inherent risks of high-intensity soccer training, including but not limited to: sprains, strains, fractures, muscle injuries, and other physical injuries. I voluntarily assume these risks and release Bravos Soccer Academy, Coach Gerardo Bravo, and all staff from liability for injuries sustained during training.
I authorize Bravos Soccer Academy to seek emergency medical care if I cannot be reached immediately, and I confirm all emergency contact and medical information is accurate and current.
SMALL GROUP CLINIC WAIVER
I understand that Small Group Clinics are focused skill sessions for 2–6 players led by Bravos Soccer Academy coaching staff, and involve tactical drills, quick combinations, and match-scenario play.
I acknowledge the inherent risks of soccer training including but not limited to: sprains, strains, fractures, and other physical injuries. I voluntarily assume these risks on behalf of my child and release Bravos Soccer Academy, Coach Gerardo Bravo, and all staff from liability for injuries sustained during normal program activity.
